How do I get a prescription refill?
Contact our office during business hours and a message will be
forwarded to the physician. Please allow at least 48 hours for
refills to be called in. Due to laws on this topic absolutely no
medications will be refilled after or on weekends.
Where is your office located?
Please refer to our location
page for the location, hours of operation and contact number
for our five locations. There is a map to each
location for your convenience.
How can I schedule an appointment with your office?
You may call at any time during our regular business hours.
Do I need a referral?
Our office does not require referrals, however
a referral may be a requirement of your insurance plan. If you
are unsure if your
plan requires a referral, please contact your insurance company.
What types of insurance do you accept?
Worker’s compensation,
Blue Cross Blue Shield, Medicare, and many others. Please contact
the member service department for
your insurance to verify our physicians participate in your particular
plan.
Will I have to pay a co-pay?
Yes, co-pays are collected at the
time of service. Your co-pay is generally printed on the front
of your insurance card. If you
are unsure of the amount of your co-pay, please contact your insurance
company.
What is your payment policy?
If you have valid insurance, we will
collect any co-pay and/or deductible at the time of service and
file your insurance. Otherwise,
payment is due at the time of service. If you have questions, or
special concerns regarding payment at the time of service, please
contact our financial counselor at (228) 392-9355.
What is your policy for releasing medical records and x-ray films?
All
patients must sign a HIPPA compliant authorization to release information
if they would like copies of their records for their
personal files, legal, or insurance reasons. This request is then
forwarded to our record production department. If you need your
x-ray films, these films will be copied. There is a charge for
this service. Allow 7 – 10 days for production of these records.
What should I bring with me for my appointment?
Picture ID and
insurance card, list of current medications, and completed patient
information forms available for download. (Make ‘patient
information forms’ a link to the patient information page)
Do you fill out disability forms and FMLA forms?
We will complete
routine forms you bring to the office. Complex forms or medical/legal
letters are completed only with individual
agreement by the physician. There is a fee associated with this
service.
Do you perform Independent Medical Examinations?
Independent Medical
Examinations are done only with individual agreement by the physician.
